When
you order online, by fax, phone or mail:
The
vital information you provide so that we may process your
order efficiently is never sold to or seen by outside companies.
This information, such as name, email address, phone number
and credit card information is used solely for processing
your order.
Your
email address:
Used solely to inform you of a backorder, shipping date,
credit or delivery.
Your
phone number:
used solely to verify credit card information, if needed
or to gather payment information or address information.
Your
name and address:
Used solely to ship your order to you.
Terms
of Service:
Lake Uniforms is not responsible for lost, stolen or damaged
goods incurred in shipping your uniforms and scrubs or related
merchandise to your business and or place of residence.
We will research the situation and find the cause of the
problem, if possible. If UPS, FedEx Ground or USPS is the
cause of the problem, you will be compensated. If your package
is delivered in error to an incorrect address, we will file
a claim with the shipper and replace your merchandise or
credit your card if the shipper is found to be at fault.
Please be aware that these situations can take up to 30
days to investigate and resolve. If you live in an apartment
complex and your packages are signed for by apartment management
and or personnel, our company is not responsible for your
order if your order is not given to you. If you believe
there is a possibility that your order may be given to apartment
personnel, inform them that you are expecting a package.
If your order is signed for by anyone, it is considered
delivered.
a) If your order is delivered to your address but not signed
for by you and is released by the delivery driver, it is
considered delivered. If your order is delivered to and
accepted by a neighbor, friend or relative or any person
or persons residing at the delivery address, it is considered
delivered.
b) We
are not responsible for the merchandise if it is not received
by you or it is received by a third party unknown to you.
We will track the package and investigate signatures in
an effort to find your package.
c) We
reserves the right to refuse any sale at the time of ordering
and up to but not limited to 15 days from the order date.
Sales can and will be refused if the billing information
used for a credit card purchase is incorrect.
d) We
reserve the right to refuse any order at any time. We provide
nursing uniforms, scrubs and accessories to the 50 United
States including Alaska, Canada, Puerto Rico and Hawaii
for additional shipping charges as well as US military bases
abroad. We also ship overseas for additional shipping charges.
If you
order your uniforms and scrubs and related items from Lake
Uniforms and its related internet sites you are subject
to the fair purchase agreement set forth Herein.
1) You
agree to give our company a minimum of 3-4 business days
to ship your "in stock" items. 97% of all in stock
orders ship in 1 to 2 business days.
2) You agree to give our company the opportunity to replace
defective merchandise sent to you by the manufacturer within
30 days of receipt of merchandise.
3) You agree to give our company the opportunity to resolve
any dispute with merchandise either defective or mis-shipped
to you within 30 days of receipt of merchandise.
4) You agree that once your order has been shipped in whole
or in part you may not demand credit or cancellation of
your order at any time there after. You may return your
uniforms or exchange your uniforms using our exchange form.
If criteria is met for such returns or exchanges within
15 days of receipt of goods. The terms of service set forth
here in have been established for both you the consumer
and us the seller and will be acknowledged and enforced
as of this date November 1, 2003 these terms of service
are subject to change at anytime.
